Office Policies

Appointments:
We do not take walk-ins. Please always call ahead and we will do our best to accommodate same day appointments for sick visits. We generally do not allow same day check up appointments.

Late and no-shows:
We understand that especially with children, your schedule can be unpredictable. Therefore, we do not charge for no-shows. But for the courtesy of other patients, please call ahead to let us know if you will be late or will not make it to the appointment, so that we can give your appointment slot to another patient who may need it. If you are late for more than 20 minutes without notifying us, we reserve the right to reschedule your appointment.

Phone Calls:
We will try to return all phone messages as soon as possible. Most morning calls will be returned by lunch time, and afternoon calls will be returned before we leave the office. Any after hour calls will be connected to the answering service who will page our on-call doctor. If you do not hear back from us more than 20 minutes after your first page, please page again.

Social Media and Email:
We do have a Facebook page, and may email you from time to time. However, we only intend to use those to provide you with the most up-to-date news and announcements, and not as a mean for you to ask us questions. Please do not try to contact us through email or through social media networks. Questions regarding your child’s health are best handled through a phone conversation, because we may need to ask you many questions in order to provide you with the best advice.

School & Camp Forms:
We are happy to complete any forms needed for school, camp or other activities at no additional charge. You can bring the form with you to your check-up and we will complete them and return them to you at the end of your visit. If the patient has had a check-up within a year, the form can be submitted by mail, fax or dropped off in person, but please allow us 5 -7 business days to get the form back to you. Please read all forms before submitting them to us and make sure you fill out the parent's section completely.

Co-payments and Insurance Cards:
Please bring your current insurance card to every visit and make a co-payment if your plan requires so. If your child is by him/herself for a checkup, you must be easily reachable by phone in case we need parental consent.

Record Transfers:
If you are transferring to our office from another practice, you can either sign a record release authorization at your previous office to have them send the records to us, or you can sign one at our office (or download a copy
here) and we can request the records for you.

If you are transferring out of our office, please sign our "Authorization to Release Medical Record" form at our office or download a copy
here. The form should be signed by the parent or legal guardian if the patient is under 18 years of age and by the patient if he/she is over the age of 18. There is no charge if you are requesting for the records to be transferred directly to another physician. However, there is a $0.25 per page charge for records released to anyone other than a physician or medical facility.

Immunization Policy:
We follow the immunization schedule recommended by the American Academy of Pediatrics. We understand that parents may have different reasons for not following that schedule or deferring certain immunizations. Please discuss your concerns with your doctor at the appointment, so that we can try to accommodate you accordingly.
